End-game dungeons would be areas that consist of a random number of rooms and each room would have a monster(s) to kill followed by a skill requirement to reach the next room. ie: "You enter the first room of the dungeon and see 40 rats... Gross, you should probably kill them all before you proceed." After you kill examine the door to the next room and it would say something like. "This door is covered in thick vines, chop them down." which would then require a random woodcutting level to cut based on the level of the dungeon.

It's pretty much the dungeoneering skill in runescape. There's a lot you could do with this.
Item/exp rewards at the end of the dungeon
make a skill associated with it
depending on balance limit dungeons to a certain number per day
make it so that "treasure maps" that lead to the dungeon drop randomly from skills and are tradeable 0.01% droprate or worse
with Raserei's suggestion of alchemy use potions to help you progress if you don't want to level a certain skill
have sub only dungeons (agility/jewelcrafting/thieving requirements)
boss at the end of the dungeon
